Adds a Host header to CONNECT requests when using SSL with upstream proxies. This header is apparently required by some content-filtering proxies, notably Webwasher.

Adds improved selection into large site maps. Previously, when one or more branches were selected, the UI would block while the table of selected leaf nodes was compiled. Now, a progress bar with cancel option is displayed while the table is compiled, and the rest of the UI is still usable.
